Evaluation Resources
All evaluation resources are now housed in Shared Files in the Evaluation Resources Folder
Resources include the following:
CLUB MEETINGS – These surveys were designed with county input based on curricula delivered in the classroom. Many of the surveys are aligned with curricula found in the Staff Only files, while others were developed at the request of a county Extension Office.

Folders within the Club Meetings folder are orgnaized by lesson title/content area.
These surveys resemble quizzes and are generally designed to be administered as posttest only. Most have a variety of questions: knowledge, intended behavior, perception, open-ended, etc. When utilized as posttest only, they measure “demonstration of knowledge” vs. “increase in knowledge.” They can be used as pre and posttests if desired.
All surveys include a Data Tabulation Spreadsheet with pre-crafted impact statements that reflect post-test only results.

PROJECT ACHIEVEMENT – These are the surveys that have been in use since 2015, not the newly developed surveys piloted in 2025 and 2026.

There is a folder for the survey for youth in grades 4-6 (Cloverleaf) and grades 7-12 (Junior/Senior). These surveys are designed to be used with youth that participate in district competition.
END OF THE YEAR (EOY) TEACHER SURVEY – This survey is customizable by counties.
