After program review and approval, the UGA Policy for Programs and Activities Serving Minors requires all programs/activities to be registered with the University of Georgia. All UGA Extension 4-H programs must be registered annually by Extension personnel. To assist with the registration, local agents and staff can reference this template and the annual approval letter from our State 4-H Leader. Next, complete the UGA Program/Activity Registration.
