COMBINATION LIKERT AND KNOWLEDGE SURVEY TEMPLATES

The following surveys are designed to be youth-friendly. They resemble quizzes and include a variety of question types: knowledge (matching, multiple choice), intended behavior, perception, open-ended, etc. They also include space for Likert-style questions in a youth-friendly format for both the traditional Likert format as well as the Retrospective Post-Then-Preformat.

PROJECT ACHIEVEMENT

The following surveys were designed to be administered after a 4-H’er participates in the district Project Achievement Contest.

 

Grades 4-6 (Cloverleaf)

Grades 7-12 (Junior/Senior)

 

 

EDUCATIONAL CLUB MEETINGS

The following surveys were designed with county input based on curricula delivered in the classroom. Many of the survey are aligned with curricula found in the Staff Only files, while others were developed at the request of a county Extension Office.

These surveys resemble quizzes and are generally designed to be administered as posttest only. Most have a variety of questions: knowledge, intended behavior, perception, open-ended, etc. When utilized as posttest only, they measure “demonstration of knowledge” vs. “increase in knowledge.” They can be used as pre and posttests if desired.

All survey include a Data Tabulation Spreadsheet with pre-crafted impact statements that reflect post-test only results.

We are hoping to develop templates for Club Meetings, Project Achievement, and Camp. These will have some generic program information included.
